Hi, I'm Carol, the Founder of Cornerstone Creative Wellness Solutions, LLC. I am so excited and honored you will begin working with us on your healing journey.  Before your first session, please read and sign the New Client Intake Form and the Informed Consent & Liability Waiver prior to your visit to Cornerstone Wellness - and take a minute to read through our terms & policies below.   ​

CANCELLATION POLICIES:  

Appointments: If you cancel, or reschedule an appointment within 24 hours of your appointment time, or do not show up for your scheduled appointment, you will be charged a cancellation service fee equivalent to the service for which you were scheduled.  

 

Classes, memberships and class packages are non-refundable. Class Passes are non refundable, non-extendable, and non transferable to other persons. They are subject to time limits. Always check your expiration date before and after your purchase.  If you are late for a class or private class, the class will end at the original time.  

Workshop and series cancellations require a 48 hour email notice before the workshop or series takes place. Cancellation requests with less than 48 hours notice will not be honored unless we can fill your space.  Workshop cancellation requests must be requested by email to: cornerstonewellness2020@gmail.com.  Absence to a workshop or series is non-refundable.  

 

Cancellations by the Studio: Classes, workshops and events could occasionally be canceled by the studio due to inclement weather or instructor illness. If your event is canceled, you will be notified by email, and you will receive a full refund or full credit toward your account if you choose.  For series, there will be one make up day built into the schedule.

SERVICE & LIABILITY POLICY: Services offered by Cornerstone Creative Wellness Solutions, LLC are complementary wellness services. Practitioners will not diagnose conditions nor do they prescribe substances nor perform medical treatments nor interfere with the treatment of a licensed medical professional. Services offered by Cornerstone Creative Wellness Solutions, LLC do not take the place of medical care. Cornerstone Creative Wellness Solutions, LLC, and its Practitioners will not be liable for a client's medical, mental or emotional conditions before, during nor after services provided by Cornerstone Creative Wellness Solutions, LLC.

COVID POLICY:  
A mask is not required for customers who are fully vaccinated. If you have come into contact with anyone who has shown COVID symptoms, or you have experienced symptoms yourself within the last 14 days, do not schedule an in-person session until 14 days after exposure, and/or after the last day of experiencing symptoms yourself. If you have an existing appointment and become ill, please do not arrive for your in person appointment and reschedule as soon as you are well enough to do so.
